*Monitoring Questionnaire (MQ) Process:*

1. The Monitoring process was streamlined and is now being administered via SPConnect for all programs that report on SPConnect.
2. ETCs will be reaching out to their sites to review the process and confirm timelines to ensure that SDS will be available to respond to the MQ once released.
3. With a risk-based approach, MQs are tailored to that individual site and therefore we recommend that each site creates a word document of their own questions to copy and paste from as they input into SPConnect. This will reduce any SPConnect frustration due to timeouts and lost data.
4. The MQ will show up on the SDS’ SPConnect dashboard and needs to be completed within 15 business days from the date that it shows up on the dashboard.
5. Once the SDS completes and submits the MQ back to MTCU, the ETC will respond back within 15 business days.
6. If further action such as site visits, etc are required, the ETC will be communicating that directly with their sites.
